Release and Waiver of Liability

 

 

By signing in to Glomedicine.com you are requesting personal yoga, health, wellness, and fitness instruction from Michelle Mendonca and/or her associate teachers at Glomedicine.

 

By signing in to Glomedicine.com, you represent that you are in good health and that you have been examined by a licensed physician within the past 6 months and have been found by your physician to be in good health and fully able to perform physical exercise. You hereby agree to indemnify, save and hold harmless Michelle Mendonca, her associate teachers at Glomedicine, her family (daughter Gloria Estrada and spouse, Ken Estrada), and her studio space at 410 Old Santa Fe Trail, Suite D, Santa Fe, NM, and/or your home and its owners and occupants from all loss, damage or injury including, but without limiting the foregoing, all claims, judgments, liens, liabilities, suit, actions, debts and obligations in connection with or arising out of your participation in receiving yoga, health, wellness, and fitness instruction from Michelle Mendonca or her associates at Glomedicine; And hereby release all claims and exculpate Michelle Mendonca and her associates at Glomedicine from all damages for personal injury or property damage which may result from your participation and all activities conducted in training and instruction with Michelle Mendonca and her associates at Glomedicine. You further acknowledge that you are aware of the risks associated with participation and activities conducted, including risk of property damage and personal injury to you, and you have fully assumed such risks.
